The sniffing position also involves neck movement that isn’t ideal when spinal injury is suspected. In addition, inserting an oropharyngeal airway is a subsequent step to maintain patency, not the act of positioning the airway itself, and it carries its own limitations if the patient isn’t fully unconscious or has a gag reflex.
